The Challenge: Severe Leadership Churn in Long-Term Care
We were engaged by a long-term care organization in Walworth County, Wisconsin, that had experienced four administrators within one year.

Regulatory & Survey Outcomes
Real leadership is tested when unexpected challenges occur. During the interim engagement, a resident elopement occurred. The facility responded promptly and implemented immediate corrective measures.
1. Rapid Incident Response & Pass Non-Compliance
The resident was located safely with no injury reported. The facility executed an immediate, rigorous root-cause review and corrective action plan. State regulators reviewed the incident, and it was accepted as pass non-compliance as of the exact date of the incident, with zero monetary penalties.
2. Low-Level Deficiency Extended Survey Results
During the subsequent extended survey, the facility received only two lower-level deficiencies related to F609 and F730. These findings included an opportunity related to abuse/neglect reporting and completion of required annual evaluations.
3. Most Successful Recent Annual Survey
The facility subsequently completed its annual survey with lower-level deficiencies, marking its most successful survey outcome in recent facility history. According to facility staff, the survey experience was viewed as “one of the facility's strongest and most successful survey experiences in years.”
The Outcome: Lasting Organizational Stability
Perhaps the most meaningful outcome of this engagement was lasting organizational stability.
After experiencing four administrators in one year, the organization successfully transitioned to permanent leadership at the conclusion of our 3-month interim engagement.
